These 2 articles discuss the use of telephone communication as a means of providing comprehensive case management services for diabetic patients via regularly scheduled "care calls" or for acute needs. Taken together, they highlight the vast possibilities of such an approach to simultaneously increase the quality and reduce the cost of care.
The first article, from Southwest Medical Associates, Las Vegas, Nev, and American Healthways, Inc., Nashville, Tenn, describes a disease management program instigated by a health care purchaser for all of their participants with diabetes. All enrolled diabetic patients (age and ethnicity not specified) were classified according to a 4-tier population risk stratification model that determined the frequency and intensity of interventions. The intervention consisted primarily of outbound telephone calls from specially trained nurses, with frequency of calls varying from daily to quarterly based on the risk stratification of the patient.
