AAP Grand Rounds 3:54 (2000) Is Device Closure of Atrial Septal Defects Better For IQ Than Surgical Closure?Source: Visconti KJ, Bichell DP, Jonas RA, et al. Developmental outcome after surgical versus interventional closure of secundum atrial defect in children. Circulation. 1999;100 (supp II) II-145II-150.
This study from the Boston Childrens Hospital evaluated 45 patients who had elective closure of their atrial septal defects (26 by surgery and 19 by device closure via cardiac catheterization), using post-repair IQ as the primary outcome variable. Standard neuropsychological testing was performed on all patients using a protocol that has been used extensively for children with transposition of the great arteries. None of the children were tested before closure of their atrial defects. Families of device patients had higher parent IQ, higher maternal education, and higher level of maternal occupation although these differences were not significant. The age at closure was the same in both groups (65.6 mo and 69.7 mo), but children with larger defects had surgery rather than closure in the catheterization laboratory. Of the 45 children, those
