At Yale-NewHavens pediatric primary care department, charts from 163 well-child visits (during September and April to balance resident experience) were reviewed for standard growth measurements (height, weight, head circumference) plotted on a growth chart and growth problems noted. In this setting, the growth measurements were performed and recorded by nurses and nursing assistants, while the pediatric residents/nurse practitioners were expected to plot them at each encounter and record relevant interpretations. For the 147 visits that fit study parameters, at least 1 growth measure went unplotted for 21% of visits; 22 of 65 clinicians (34%) failed
