Question: Among children younger than 2 years with their first UTI, does obtaining a VCUG in those with normal renal ultrasound improve outcomes versus observation?

Investigators at the Santa Clara Valley Medical Center in San Jose, CA, conducted a retrospective study of infants younger than 2 years with a first febrile urinary tract infection (UTI) to evaluate the efficacy of a clinical algorithm limiting the frequency of voiding cystoure-thrograms (VCUGs) and reducing routine use of prophylactic antibiotics. The algorithm was implemented at their institution in 2008. With the algorithm, VCUG is limited to children with an abnormal renal ultrasound (RUS) or an atypical UTI.
