The first cases of HIV infection in the United States were recognized in 1981.1 Progress in the understanding and treatment of HIV has been astonishing since that time. The epidemic blossomed in the United States in a then-marginalized population; time and subsequent events have taught us much about HIV transmission and treatment, and brought about a sea change in majority public opinion about gays and lesbians.
