Investigators from multiple institutions conducted a prospective observational study to develop a prediction rule for identifying febrile infants at low risk for serious bacterial illness (SBI). Infants <60 days old presenting to a study ED with a rectal temperature of ≥38°C were eligible. For all patients, treating ED physicians recorded their clinical suspicion of SBI risk and provided an assessment using the Yale Observation Scale (YOS). A complete blood count with differential, serum procalcitonin, urinalysis (UA), and blood and urine cultures were also obtained from all patients, while cerebrospinal fluid was obtained at physician discretion.
